Help - How to get out of wraith place?

How do you get back to the surface after you kill the last wraith? I went back with the eyes and heart and killed the wraith, but I can't get back to the surface.

Just climb up the ladder behind the Nilfgaard's body.

I did climb the ladder to the room above. However, I can't climb up the entrance to that room.

Once the fire is out, there is another path from that room to the entrance. Go up the stairs and explore the area to your right.
