PM I'm searching the south refugee cave in the swamp cemetery, but I can't find Zdenek there.
Is there anything I've done wrong?

That cave has two chambers: the refugees are in one, and Zdenek is in the other. Explore the whole cave, and you should find him. Hold down the Alt key (or turn on Show Object Names in the Options) to make it easier to find him in the dark.

There are 3 different caves full of refugees, so make sure you're in the right cave. You can find a map showing the location of the cave here.
You need to have defeated the guy's predecessor; if you have, your journal should say,
" The Rock Defeated
I beat The Rock. He claims that only his teacher is stronger, and he lives in the cemetery located in the swamp beyond Old Vizima. I must face The Rock's teacher in the swamp cemetery."
If your journal says that AND you're in the right cave, then yeah, it sounds like it's a bug. Seeing things in the caves can sometimes be a problem -- did you hold down the Alt key to show object names?
