PM So I ended up killing everyone in the Salamandra based in the swamp before starting the diplomacy and hunting quest. The Witcher Wiki says this quest is now impossible to start - am I now unable to complete the entire Leeuwarden quest line?

Did you find the Encrypted Document on Roland Bleinheim's corpse? If you have that, then the rest of the chapter should proceed normally; all you'll miss is the quest XP (and the privilege of fighting alongside the Hierophant and his wyverns
