Okay trying to finsish Declan's quest and place his friend's skul(s) I have 123 of them in teh sarcophagous which I do without any problem BUT no ghost no cutscene no leo and no ****ing red meteorite.
Any clues and yes i ahve tried this 10x and yes i have removed the override file and no it doesn't work

I discovered the cave early in the act and as (after Wi2) my combat was pathetic to say the least) I used the echin whatevers to practice as they spawned nonstop and I gained 125 then 120 then 115 then 110 then 100 then 95 etc expn off them. The merchant i never found until I noticed that i had missed a quest and when I found him I just walked away and left the in then came back. But I was suspicious that this would break the quest, so I went back to the cave and knocked off one more and then told him. I did the internment before and after talking to the priest(or whatever) and it made no difference. I am really p*ssed as I wanted that 3rd red meteorite and it took me quite a few goes to get the first bit from that bloody great praying mantis..

I don't know how you would have been able to do the internment BEFORE talking to the Reverend, since the door to the Chapel is supposed to be locked until you talk to him. It makes me wonder if you've got a REALLY big bug or if you put the remains in the wrong place.

And you'll want the experience from the quest, too.
What journal entry do you have for this quest? Where does the game think you are in solving this quest?
You should find the sarcophagus in the basement of the Chapel. When you click on it, you should get a pop-up box with the gift action open, like this:

When you put the Human Remains quest item into the sarcophagus, the King of the Wild Hunt should immediately appear and begin talking to you, like this:

If all of this sounds familiar except that the KotWH didn't appear, upload your game somewhere (zip it first, please), give me the URL, and I'll look at it for you.
If this doesn't sound familiar, then you may be going about things wrong. You need the quest item Human Remains, and they go in the sarcophagus that's in the basement of the Chapel.
Depending on the conversational path you take through the conversation with the KotWH, you may get some important foreshadowing for the end of the game, so I wouldn't want you to miss it. If you aren't doing anything wrong, and if your game isn't TOO broken, I should be able to force the conversation to start, so you can see this important dialogue (and also get your red meteorite, which seems to be what you care about).

I only did the internment after talking to the reverend but I tried it both before and after talking to him about the salamandra: it makes no difference except that if I do it after there is no merchant in the inn as expected.
I might try and use a cheat engine to drop the skulls down to one but I think that I have really broken the quest by entering the cave before receiving the quest. TBH I didn't even know there was a quest until I checked a walkthrough before I moved on to Act 2 and I had to go back a to an earlier save. It is rather STUPID that such an importnat quest depends upon the vagaries of catching that merchant in the inn and not outside as some quest articles state and only during the daytime.
I do get the screen where I place the skull(or skulls)but nothing happens afterwards and I have tried 20 times or close.
I will try and zip a save but TBH the only way out i see is to go back to a precave save which serves me right for being so obsessive about experience and combat.

Okay i have sorted it out.
IF you have one skull in yr possession when you receive the quest then walk away come back and tell him you have it.
No matter how many echinops you kill tehre will only be one skull.
IF YOU DO NOT, and go back to grinding in the cave the skull just becomes an unlosable loot item in your quest item folder and if you place it in the sarcophagous(1 or the lot) it is NOT recognised as being the desired quest item and hence no sequela.
I went back to a level 5 save and was so depressed that I thought of chucking in the game then I noticed that before I had the quest at level 9 i had only one skull so I just did the above and Voila! cut scene and poor LEO on WHOM YOU must USE FAST not slow as he *blushes* err "got" me the first time.
BUT I have my 2nd piece of red meteorite and I know where the third one is.

Glad it finally worked out for you! Yeah, you need to put the QUEST item in the sarcophagus. I had no idea you could even GET more than one set of Human Remains.
CDPR really should have put the echinops with the quest item in a separate spawn set and only activate that spawn set after you talk with Leuvaarden, so this couldn't happen. But you're the only person I've ever seen report this problem, so I guess it hasn't happened to too many folks.
At 54, I'm a little old to be a medical student.
I'm a licensed psychologist and used to do therapy, though I've been too ill to practice for the last couple of years. It's still one of the healing professions, though, so you were in the right ballpark. 
And about grinding -- you can grind in The Witcher if you ENJOY grinding, but one of the things I love about this game is that you never, ever HAVE to grind. If you do all of the main quests and most of the side quests, you'll have enough experience to handle whatever the game throws at you. If you ENJOY grinding, feel free to continue doing it, but if you're just doing it because you want the additional levels, you don't have to do that. The game is very nice in that way!

Thank You....Actually I am also retired as the end product of that which I accused you.
I really don't enjoy grinding(life is becoming rather short) but I found myself so useless in the combat that I had to do so to learn.
Now on the second run through most of act 1 I cannot be bothered as my main aim is fulfilled.
Funny thing RPGs....kept me alive through the bad years...I dont think that their postive escapist value has ever been really appreciated by the "experts" of this world. Note that I have much contempt for such people as most really have no clue about the grey world which really exists and not some convenients framed black and white one.
