1. Spare Aryan - Aryan himself is a prick and doesn't have a lick of sense as a ruler, but this option is better than his mother taking control and selling out to nilfgaard. Also, as some have pointed out before, Gerald is a Witcher, not a noble, sorcerer, or a pawn of politics. His goal is to mainly serve his own interests i.e. get his memory back, be with Triss, find Yennefer, etc. So therefore is no reason for him to kill Aryan.
2. Side with Iorveth - during the first chapter when you have the option of either giving Iorveth a sword or helping Roche I sided with Iorveth. While Iorveth's ideals are slightly anti-human his overall plan to create an independent state where the elves can rebuild their race and be friendly to both elves and humans is a great idea. While geralt is not overly concerned with politics as a whole he has repeatedly expressed the desire to protect everyone's ability to choose and be free, seeing as he himself does not like to be maneuvered against his will. Iorveth, while slightly anti-human, does not kill Roche when he gets the chance, even though he could have whereas Roche would gladly kill Iorveth given half the chance due to his racism. I also find Roche to be an unbalanced individual with little in the way of personal emotional control. At least Iorveth can put aside differences and act for the greater whole when need be, as shown by his support of the equalist ruler Saskia. Also, even though there is an uprising in the town and the humans slaughter most of the non-humans Iorveth let's it go in order to travel to Vergen Vs. leading his forces against the town in revenge.
3. Save Mottle - Again, Geralt usually shows his compassion to the sentient non-human races and treats anyone who is non-human but overall neutral or good with respect to their choices (this is evidenced by the fact that he says early on in the game that Witchers do not hunt dragons and the fact that he states later that he would spare the troll at the bridge as long as he was not evil). He would be unlikely to let the elves perish due to Loredo's cruelty. He would love to get even with Loredo on a personal note, but saving the elves and letting someone else take care of Loredo later seems like more his style (granted this is my assumption based on the fact I have not played the first game or read the books).
4. Save Stennis - Geralt is not a political player and could care less about what really happens to stennis, whether he poisoned saskia or not (he is unaware that she is a dragon at this point so you can't take into consideration what he would have done is stennis was guilty of trying to poision a gold dragon). However, he makes a point to be fair and allow stennis to have a fair trial later so that saskia can be the one to judge him. Stennis may be royalty and an ass, but saskia is the defacto ruler of vergen (it's entire populace and fighting force back her rather than stennis) so she should be the one to decide his fate (even though she is under the control of philippa for right now she won't be later and can decide what to do about him then).
5. Spare Henselt - Henselt is a douche, but he is a royal douche and a king. If Geralt kills him to save saskia when he can get the blood without killing him and still save her then there is no reason to kill him. If he did kill him than Geralt is no better than Letho. Plus Geralt has no personal reasons to kill Henselt. Henselt has done nothing to him, has not harmed anyone he cares about, and has no information about Geralt or his past. He doesn't even recognize him as being the white wolf and allows him to leave camp peacefully if he agrees to lift the mist (which geralt plans on doing anyway).
6. Save Triss - Going after triss and saving her from Letho and nilfgaard has been one of geralt's primary goals ever since they left Flotsam. Saving her is more important to him than any political machinations and he could care less about saving Philippa or curing saskia. As long as saskia is alive there is a chance of Triss helping her cure saskia whereas philippa is likely to betray geralt later on for her own purposes as soon as she gets the chance. Also, with triss saved you have a credible witness against the Lodge and a targeted witch hunt against the Lodge rather than the sorceresses as a whole, restoring the conclave and the council while taking out the majority of the lodge who were used as tools by nilfgaard to assasinate many of temeria's rulers.
7. Let Sile die - let's admit it, she's a witch with a capital B and does not deserve to live. She is just trying to save her own skin and the information she offers about the location of yennefer can be gained from letho or by geralt's own investigation. That information alone is not enough to pardon her for her crimes or for her attempts to kill triss. Geralt has a personal grudge with her and is more likely to eliminate her for his own reasons rather than for any political ones. Plus letting her die is the death of a another member of the lodge, therefore weakening the individualistic sorceress lodge and strengthening the position of the conclave and council.
8. Spare Saskia - saskia is a gold dragon and an inherently good one at that. She is fair minded and peaceful at most times, plus she saved Geralt's life, a favor for a favor. Also, witchers don't hunt dragons. He only fights her to defend himself and she only attacks because she is under the control of the lodge. Once sila is dead and philippa blinded there is not much of a chance that she will remain under the control of the lodge for much longer. Triss will likely hunt down philippa on her own later on and finish her off or someone else will. Either that or geralt and triss will find a way to cure her of her mind control.
9. Kill Letho - Geralt wants to clear his name of Foltest's death and pay letho back for everything he has done both to geralt and to triss, let alone all the things he has done on behalf of nilfgaard to the north. Even if they were comrades in arms once before that has no bearing on whether or not he should be spared now. Sure, they traveled together and letho helped geralt find the wild hunt and yennefer, but that was only because geralt saved letho's life. Letho was just paying back a debt, like any true witcher would. Geralt owes him nothing and letho condemned himself by getting involved in politics and king slaying, he is no longer a neutral witcher but a pawn of nilfgaard and should be taken out.
Okay, so a summary of what happens based on these decisions. You have two rulers of the north with one independent state. Henselt and Stennis rule most of the north, but saskia's rule of Vergen is recognized by Henselt and therefore Stennis must abide by it. He may decide to attack later to try and regain control, but Saskia will have the support of the final surviving lodge member, philippa, and an extremely large following of most of the non-humans in the north, so there is little chance stennis will be able to do anything about it. Stennis and henselt, while originally on opposing sides, were able to come to an agreement about who should rule what, so should be able to come to an agreement later on about working together with saskia to fight against an invasion by the empire ( the enemy of my enemy is my friend, as the saying goes). So you have the conclave, the council (which will help steady and support the growth of the north) Three rulers who can put aside differences and work toether, geralt gets most of his memory back, saves triss, clears his name, gains favor from stennis, henselt, and saskia for sparing their lives when he could have killed them, and gets information about the wild hunt and the possible location of yennefer. The only downside to this is that philippa is still alive and saskia is still under her control. This may not turn out to be good later on since philippa likely has a grudge against geralt for leaving her to rot in the dungeons, but we will have to see how that turns out. there is a chance to cure saskia later on, so that is a least an option. Overall I think this is the better conclusion for the north and still remain true to geralt's personality. He was neutral by not killing any of the three possible rulers, didn't get overly involved in the politics for his own gain, and was able to accomplish most all of his own goals in the process while staying true to the witcher path. Anyway, just my opinion, everyone is free to comment/oppose/enforce these choices as they see fit if they have additional information I missed or information from the first game or literature than might influence these choices that I don't have since I have not read/played either of them. I am eager to see how these decisions effect the third witcher installment if it is also released for a console system.

I wonder if that would change the options on any of the other choices though or how any of the non-humans react toward you? And it's not so much that I am trying to cause the least amount of death/pain possible, because quite a few people die in the options I selected on my first playthough (even though most of the people who died were evil and/or politically incompetent). I initially chose to give the sword to Iorveth because I didn't know what would happen to him at the time if I left him bound and didn't give him a weapon. Now that I know about it though I'm likely going to go that route this time around.

What is the best depends who you are rooting for. After a lot of playthroughths and seeing practically all of the details I figured that the most beneficial way for the North is to play Iorveth path, save Stennis (he may be persuaded to collude with Nilfgaard later, but no king at all is still better for Nilfgaard - no need to persuade anybody), Henselt is alive, Temeria is not in chaos but is parted between Redania and Kaedwen. In this case an impact of assassinations is practically not-existent. Save Triss and the mages, and spare the dragon. Even though you do not save Saskia from Philippa, Saskia is alive. Perhaps Phillipa is still in the dungeon, and I hope Radovid will kill the bitch.
I personally root for Nilfgaard, and when I am done with the North, there are no kings in Aedern and Kaedven, uprising is crushed, Saskia is dead, Temeria got Anais, while Arian is dead and can't defend her, and her mother is still probably in Nilfgaardian custody. Thus Temeria is pretty much ready for a civil war. Lodge is destroyed, and mages are hunted and persecuted all over the North. Letho, my bro witcher, goes south. After the job well done nilfgaardian delegation returns home to enjoy a well deserved vacation with their friends and families. The emperor is happy with an outcome, and that is all that matters.

Well, Zoltan tells you that Loredo is looking for any excuse to kill off the non-human residents of Flotsam, and that one more provocation from Iorvith would do it. I don't think it's too much of a stretch for him to think that Iorvith's escape would count as such and lead to said killings.

1) Geralt: The Terminator
a) Kill Aryan AND his people: They started, we finished. And we saw them off with pride, dying as free men.
c) Follow Roach: The man saved our lives, gave us food and ale while still in prison, believed in us, and his character is epic, mostly in quotes and mission planning monologues. Plus, we need to take care and watch the back of Dandelion, who became his spy even during the previous title.
d) Kill Henselt: No one touches my girl and lives to brag about it.
e) Rescue Annais: This is the only way to get the best steel sword in this game. And i NEEDED to see "Uncle Dethmold" get this ending, HIS ending.
f) Give Annais to Natalis: The right hand man of Foltest. Long live his memory, even if he hardpressed us under his service that much.
g) Kill Sille: Never liked her, she looked suspicious from the beginning; let alone insulting and bad-mannered beach.
h) Kill the Dragon: I always was expecting an epic dragon trophy, dragon scale ingredient and the rumored list of "most rare alchemical items obtained from killing one". Too bad those entries were forged; CDPR april's fool on our asses well done.
i) Kill Letho: Always has been his damn fault for all of our troubles in 2. Especially since he had been a close friend, all his deeds were twice as unacceptable and unforgivable as others might think. The viper school is terminated permanently, with only 1 mysterious and as of yet unintroduced, final member, remaining in life and on the path.
So we have an independent Temeria with a future queen who likes swords, an independent redania with adda for queen (previous game save import), a kaedwen without king or sorcerer and an aedirn in chaos with terminated royal bloodline. A weaponless lodge with philippa on the run, sille torn to bits, all mages and witches persecuted, the order starting an inquisition, all dragons most possibly extinct and the Nilfgaardians having done the maximum possible damage to the north, successful on all of their missions.
2) Geralt: The communist/socialist
a) Spare Aryan: Innocent boy, Foltest wanted him to surrender instead of dying, too.
c) Follow Iorveth: He was played for a fool. Helping him helps us. Plus, Zoltan is his friend, soldier and collaborator, so we need to take care of him and watch his back, too.
d) Save Mottle: We are not insensitive bastards, to let them die helplessly and gruesomely as bait and pawns of schemes and skirmishes. Plus, the secret key and the secret reward (mmm...), were great!
e) Spare Stennis: We need to prevent Aedirn from collapse; Demavend is dead. His son, even if a clueless idiot, must survive in sake of the country and the greater good. Plus, if you decide to spare him, you get to know that the damn bishop was mostly at fault for the attempt against the virgin's life (probably she refused his dirty advances or something, goddamn clerics).
*Remember to kill everyone involved in seeking the prototype megascope*
f) Save Triss: We need to stop the Nilfgaardians, who are the real enemy. This way, we prevent the witch hunt; we are neither fascists nor fanatics, we need to ensure coexistence and see justice prevail and served. Plus she is our woman. First the women, the children and the weak; then all the others.
g) Spare Sille: She was played for a fool, too. Plus the lodge would probably stop scheming and fight on its own against the imminent invasion.
h) Spare Letho: This man had been our close friend. He explained everything, drank with us, told us where to find all we needed, and left in peace. All people make mistakes; correcting and making amends for them is what trully makes us different from beasts.
Temeria is divided between redania and kaedwen, flotsam became Henselt's, redania is independent and strengthened, kaedwen is independent and strengthened too, aedirn gets a new king, the lodge survives but exposed and retains all members and the dragon except for Triss, the magi establish their state in loc muinne, and most importantly, all of the Nilfgaardian plots and agents get soundly foiled, murdered and terminated. The only real loss is Temeria and its independence, but at least it did not fall into Nilfgaardian hands. Better the boot of Radovid and the snout of Henselt combined, rather than 1 day of servitude under Emhyr...
