I entered gramps hut before doing this quest.... it triggers the cannibal quest instead of giving me a pilgrimage quest. I read on the wiki I can't do it any more. My question is how much do I miss? What does the quest give you? Does it have any major implications in the game?

Minor impact, at most. If you lead Gramps through the Three Islands area (west and south of the trail), he will teach you the recipe for Maribor Forest. He knows a few other things that you can best learn by keeping him alive. But nothing that has any substantial effect on the game.
