That's a hard question to answer to, much of the skill that comes into play in TW2 on Insanity is reflexes. I'd recommend a Sword/Alchemy build but with improved Quen as well. I'd also recommend farming the nekker cave in Act 1 quite a lot, it helps teach you.

That's somewhat true. There are two ways to overcome the challenges in Witcher 2, Insane mode. One is skill, and the other is abuse. Abuse is exponentially better at overcoming challenges than skill is, but the trade-off is that for some sections in the game you can't abuse, and you NEED skill. E.g. the Kayran and Letho. You don't need much skill for these fights, but you need some.
Do you find improved Quen to be much better than unimproved Quen? It's gone in one hit regardless of how much time you have left. So 40 second Quen is the same as 2 minute Quen. Also, since you're in this thread, how the hell do you dodge the Dragon's tail swipe? I had no idea how to dodge it. I was playing on Insane, but it didn't matter because I realized that I'm incompetent, and so I walked into the fight with 1200 vitality.
