I have installed patch 3 and when I die (I am fighting the Kayran) you cannot select the yes or no option to reload last saved game.

Happened before patch 1.3, too. Just hit ESC, Load Game, and pick the game at the top of the list.

It only happens irregularly. The Kayran fight just seems to be one of the more likely places for it to happen.

Just don't play on insane difficulty level, though. You could never reload any savegame in that mode. In fact, your savegame is automatically deleted if you die. Hence the "insane" name tag.

If yo are seeing the game over screen where you can normally click Yes on no to load and are not seeing the Yes/No just hit ENTER and the game will load.
