I think yes, you can spare him though I never did. He asks you if you would let him take GM, you don't let him, you fight him. I don't know what happens if you agree to give GM to him. I assume you don't fight him then.

I give him to the KotWH one time and only then because i was so low on health there was no way i could take him on but if i remember correctly you skip the fight with the hunt and go straight to the ending with dandelion

I tried that once too, to see what happens. He said something like "wise decision" or "yet another soul you hand to me" or something, and faded away.
In suspended animation.

That the leader of a pack of warrior souls knows Geralt intimately tells me that maybe that was where he went after he died in Rivia.  Geralt somehow escaped or was sent back.  The Wild Hunt was never a joke.  The King of the Wild Hunt's role will not be small.

It's been awhile since I played, but I got the impression that Geralt was somehow doing the bidding of KotWH without intending to.  All those people you killed were part of KotWH's plan for Geralt.  Kind of like the movie Angel Heart where the detective realizes he's been working for the devil and he'd actually committed the murders he's been investigating.

The KOTWH is death, he mocks and makes Geralt doubt himself so Geralt will feel that he must prove himself... by slaughtering more people, KOTWH will not kill Geralt outright because Geralt serves him unwillingly, because his path is paved with the corpses of his enemies. He sends so many souls to the Grim Reaper, KOTWH wants him alive as long as possible to feed him more souls.

The only way for Geralt to free himself from The Wild Hunt is to lay down his arms and vow never to kill again, a' la Rurouni Kenshin
