How to avoid getting stuck in act 4

Well, I just got stuck after killing Dagon, like many others.  I backtraced my save games trying to find a point where something would go differently and allow me to continue to the finale. And I think I found the point:
After talking to Berengar, there should be a dialog triggered by Alvin when entering the Shore region, where he says he want to be a witcher.  This puts "witchers" step in the "Alvin" questline. If I have this step, I don't get stuck after Dagon.
When this dialog never happened, I was still on "new information" in the Alvin questline, and I was getting stuck after killing Dagon. I did not find any way to fix things any more - had to go back to an earlier save. There is no way to trigger this dialog with Alvin - it has to happen by itself.

Prance around a bit before kiiling Dagon. I got that dialogue with Alvin much before I went to kill Dagon. Do other quests. After kiiling Dagon you cant complete any more quests in this chapter, except the main one.
In suspended animation.

Actually, if you do not go back to The Lady of the Lake for the final quest reward, you can continue with Act 4 quests. My quest entry for The Paths of Destiny states The Lady of the Lake promised me a gift. I need to be patient.

I had killed Dagon very early in the chapter and almost freaked out but, I found out that her final quest reward is what triggers the last quest in the chapter.

Also, if you are a packrat, you may want to check your silver sword (or rune sword, if you upgraded it) with the Innkeeper before your final meeting with the Lady of the Lake. Otherwise you must leave one of the swords behind.
