Personally, after the opening cut scene in Act III, and then talking to Triss, the first thing I have geralt do is teleport to the tower and finish the tower quest by talking to Kalkstein. Possibly that chat "opens" the tower ?

I'll load an old saved game and check ... i'll let you know :)

UPDATE: seems to be completely normal behaviour ... you have to use the teleporter the first time :)
