I really recommend reading books written by Sapkowski (best to start with "Last Wish").
Will depend on the translation. So far i haven't seen anything as good as original.
Well, a Polish friend of mine told me that the German translation is really, really close.

Well, in chapter III at the party, I noticed that the "female companion" said "I heard witches are all baren"
Now what makes this funny is, that the model they used was the witch from the first chapter.
Also, I noticed sometimes the big thuggish guys in the temple districts (looked like the chapter 1 innkeeper), cursed humans for punishing dwarves. Sure the voice track was meant for a dwarf.
In the blue eye quest, the queen of the night, well looks EXACTLY like Carmen. Now this is weird because she is the one that helps point you to the whore house in the first place. When I was playing- I got very confused.
Yea, funny things happen when they reuse models...

In Old Vizima there's a house (there may be more) that has stairs down that are labeled "Stairs up" and vice versa (so if you go down, the stairs up are labeled "Stairs down"), it's the house you see on the left when you're facing the blacksmith's house.
Also during the ending Adda (well, depending on how you played of course) says something like: "This is more fun than a reception at the new Naracort"

English version:
Some elves at the swamp camp say (sing?) "First we take Vizima, then we take the world". That's Leonard Cohen.

At the end, before you fight the King of the Wild Hunt, Geralt says something like, "thank god, I thought you'd to play chess", a reference to Death in the Seven Seals...
Lol, so it was.. I didn't catch on until you'd said although I do know the subject matter.. cool. :)

In Act IV in the fields. Talk between Anchorite and Geralt about the holy grail. When the Anchorite explains the "crazy" conspiracy-theory, he relates it to Bronnze Danny (Dan Brown) and citates statements of the book Sacrileg (Sang Real / Holy blood and a mysterious doughter of the majoran (!!!) cult)
Great, I love that stuff
