In the Flame That Cleanses, I have met White Rayla and proceed to the hospital. Along the way, I confront a fight between the Order and Elves. Engaging this fight, I assume it will end and I proceed. However, it simply continues as more elves and Order guards replace the dead. The back door to the hospital is, and remains locked. The front door does not acknowledge any attempt to enter. What gives? Is there a bug, or have I failed to trigger an event that will unlock the hospital door. I have retrieved the contracts from the board out front as well. No help.

It's the entire quest including the phases. At one phase you don't need to enter the hospital anymore but fight the Scioa'tael in front of the hospital.

Thanks for the advice, but no dice, if I understand your points. I am now the proud owner of 70+ (that is not to say I killed all 70) squirrel tails and all White Rayla can say is "Don't cheat, the wounded don't count, just the dead". The back door to the hospital is still locked. Sorry to be so thick, but I'm stumped. I have made no "changes" to anything else since entering Old Vizima. I went back a few saves and I don't have the conracts from the board either any m ore. Any helpful hints?

I think i know the bit you mean, only with me i was with Zoltan because i sided with the squirrels last play through. I ended up with 200+ remains in that area, just for the fun of it. You need to find a certain point that progresses the quest, with a cut scene with Rayla (with me it was Zoltan)
This is actually one of the funniest dialogue moments in the game for me, Zoltan lets out this funny "roarrrrrr" as a signal to other scoia'tael, and Geralt makes fun of him, saying he will attract the she-tigers. He makes this roar again sometime later, i love it :D

Did you already met the "false Striga"? It's acutally a mutant. After you killed it you return to the hospital, talk to Shani. You should get access to the hospital (front door) even if there are fights in front of it. When you are inside, an unit of Scoia'tael enters the hospital. You have to fight them inside the hospital to protect the injured patients.
Then you have to leave to fight more Scoia'tael outside who threaten the White Rayla. This is an other phase of endless flow of new enemies. You finish it when you kill that Scoia'tael who is fighting with Rayla, could be a bit hard to find her in the crowd. When you kill her opponent… it's over :) Rayla grumbles something she didn't really need Geralt's help but nevertheless invites him to talk to him at the Order's post. And then… you make a new decision ;)

Thanks for the ideas, but still no progress. I collected over 200 tails and still no cut away to discuss with Rayla. Haven't seen the Striga, probably since I have to speak to Shani, which has not happened. I have gone over the entire map and entered every door in areas not blocked by barricades. Fighting continues in front of the hospital, whose doors are still locked. I don't have Zoltan since I sided with the Order. The only thing I have not been able to do is defeat the Scoia'tael in their headquarters, which I don't think is possible with their numbers and my current weapons. I even started over from the point of the castle with the same result. This is a great game, but it is very frustrating to get this far and be unable to finish. Any other ideas would be greatly appreciated.

On the first occasion Rayla wants you to talk to Siegfried who is injured at the hospital. Geralt and Rayla fight their way to the hospital and collect the squirrel tails. At this point you should enter the hospital. Once I had a glitch that I couldn't enter the hospital because just in the entrance / door there was a fight between two opponents. They were in such a weird position Geralt couldn't pass and couldn't attack but at any rate the door was open and accessable.
You don't have to defeat the Scoia'tael headquarter or leaders at this phase of the quest.

I've gone to the hospital repeatedly. Did so as you discribed. The back door remains "locked". The front door is closed and does not even acknowledge my attempt to enter. That is the main reason the game will not progress. I even received the message "...it's time to see Siegfried" after collecting 20 tails. That should be released the back door. I'll take a cheat code at this point. I either did not trigger something earlier in the game, have not triggered something in Old Vizima, or the game has a bug. The only change is that Rayla now says "Stay close to me" instead of "Don't cheat, the wounded don't count, just the dead". Thanks again for the help.

Problem solved ;D....for now. It pays to be persistent. Took another tack and got into the hospital. After entering Old Vizima and meeting Rayla, I moved onto the initial fight with a band of a few Scoia'taels. As soon as they were all dispatched, I moved directly to the hospital. I did not pick up any tails, I did not Pass Go, or collect $200. The back door was still "Locked", but amazingly enough, the front door let me in. Very nice. Anyway, I'm onto the Striga.
