The nurses ask me to escort them to the field hospital, so I "clear" a path to the back door of the hospital. I turn around: no nurses! So I go back, and they're just standing there, with 3 arbalesters around them. The problem here is that after killing 1 and going for the second, another one already comes out of a door, and they're 3 again! So if these friggin' nurses are waiting for me to kill these arbalesters, well, ... I've already killed a few dozen (yes, a FEW DOZEN) and there's still 3 of them! So why can't they just follow me when I'm clearing a path, instead of waiting until I killed every single soldier?

Tried exiting the area and re-entering?
Also, group style or igni helps.

It is tricky, but totally do-able. Have you tried putting Geralt between the nurses and the arbalesters ?

i tend to use igni a lot by then (it never harms "good guys" so don't worry) so i can take out the arbalesters with one shot mostly

I find they only stop and take the scared stance if  Geralt stops to fight.

I just run straight to the hospital ignoring the fighting and they follow me.

The fighters will soon find someone else to attack if you leave them alone.

If I'm not mistaken you really need the nurses to follow you I don't think you can just run ahead and clear the way

I've never been able to clear the way, tried but respawn is too fast.  So now I don't even draw a sword, just run straight to the hospital.

that is pretty much my m.o. as well (but usually i just can't resist letting off one igni)

and this is not path specific really, for all three paths, just getting thinesellf to the field hospital poste haste is the mission. Dilly-dallying as you try to kill more "bad guys" is pointless for that part of the Act

Not sure, but I think you do have the clear the path for them as you get a quest update after killing some bunches of enemies along the lines of "the path to the hospital is now clear" or somesuch.
